攝護腺根除手術後之尿失禁，往往困擾著攝護腺癌的病友們。過去研究關於影響攝護腺根除術後漏尿的因子，包含了年齡、BMI、攝護腺大小、膜尿道長度membranous urethral length (MUL)、是否做過經尿道攝護腺刮除手術、腫瘤期別、格里森分數、PSA，以及術中對於尿道之長度保留、神經保留與否、前後支持結構之保留及重建，皆被視為可能影響之預後因子，然而此類研究發現多著重於早期禁尿，且手術方式對於禁尿之影響也多在6個月至一年內。此篇研究為一系統性之回顧，針對解剖構造預測因子是否影響長期（術後12個月以上）禁尿功能，以術前之核磁共振影像為判讀標準，評估膜尿道長度及厚度、攝護腺以及尿道之角度、攝護腺尿道之長度、提肛肌厚度，以及攝護腺是否內凸至膀胱，分析1146位病患，發現膜尿道長度影響了病患的長期禁尿功能，其中5篇研究更在多變項分析看到此結果，其他解剖解剖構造預測因子則較不影響長期禁尿。根據此研究，在術前與病患分析其手術後之尿失禁風險與術前核磁共振之膜尿道長度息息相關。

There are many risk factors associated with post‐prostatectomy incontinence (PPI). Including the patient's age, BMI, prostate size, membranous urethral length, history of transurethral resection of the prostate, clinical stage, prostate‐specific antigen (PSA), Gleason score, and the surgical technique including preserving or reconstruction method. This study is aimed to figure out which anatomical predictor affect long term PPI. The study included 1146 patients, and found the greater MUL on preoperative MRI is associated with lower risk of UI 1 year after RALP.
